Sage Therapy LLC 2022
Therapy Session: 60 minutes: $165
Standard Therapy Session: 45 minutes: $125
Missed Session Fee w/o 24-hour cancellation or no show or late cancel: $75.00.
Fee for Paperwork Completion:
$65.00 per 30 minutes
-Fee for Legal Deposition, Request by Client to participate in a legal proceeding, Subpoena for legal disputes involving the client: $1000.00 an hour.
*If you are receiving gifted services at the benefit of a 501(c)(3) or Non-Profit you will not be charged for the therapy session, but must maintain a card on file for any missed appointments or late cancels at your own expense. *
Client Credit/Debit Card Payment Authorization & Acknowledgment of Payment Policies

Each client is required to complete the payment authorization form before services can be provided. Session fees (full fee, sliding scale, or co-pay) will be charged to the card at the end of each session.

If you have missed an appointment without giving the required 24 hours’ notice of cancellation or arrive 15 minutes or later to a session, $50 will be charged to your card.

If you made prior arrangements regarding payment, but payment has not been received by the agreed-upon date, your card will be charged for the balance owed.
**Fees are set for the 2022 Calendar year. If you and your therapist agreed on different fees for services rendered, an addendum will be added to your account with the agreed-upon prices. Prices are subject to increase at the end of each Calendar year **
